Endometriosis diagnosis: why it takes 7 years — and how to shorten it

Symptoms are normalised ("periods are meant to hurt"), pain is often treated with the pill without further investigation, and superficial disease is invisible on standard ultrasound. Many women see 4–6 clinicians before an endometriosis specialist is involved.

A specialist transvaginal ultrasound performed by a gynaecologist trained in endometriosis mapping can detect endometriomas, deep infiltrating disease of the bowel or bladder, and adhesions. Where deep disease is suspected, pelvic MRI with rectal contrast further defines the extent. Neither imaging modality reliably picks up superficial (peritoneal) disease — laparoscopy remains the gold-standard confirmation.
