What the data actually shows
The FDA extended the Gardasil 9 licence to age 45 in 2018 based on trials showing meaningful protection against the seven high-risk strains women in this age group are most likely still to encounter. In the UK it is available privately from around £450–£600 for the three-dose course.
Even in women already exposed to one HPV strain, the vaccine reduces the chance of picking up a new high-risk strain — and, crucially, cuts the risk of recurrence after LEEP by roughly 60%.
Who benefits most
Women who have had a recent LEEP or LLETZ, women with persistent HPV on annual monitoring, and women re-entering the dating pool after divorce or bereavement all show measurable benefit. It is not useful for treating an existing infection — but it protects against the next one.
